Can someone tell me how a coin gets this damaged and if this occurred at the mint or while in circulation?

If it happened at the Mint, it would have been in their parking lot or someone was trying to make change and couldn't break a quarter...Post mint damage...value 25 cents

That's an ouch! It could have been anything that caused this, but I vote garbage disposal.

Post Mint Damage ( PMD). This damage could have occurred in a million different ways, but damage is damage. It did not leave the mint looking like that. No premium I'm afraid.
