Yes struck from a tired die--looks like the post used to create the center of the mintmark was broken off by this point as well.

Keep in mind this was the last year for the swollen master hub on this year.
Even Machine Doubling was pushing the devices way out on the rims:

The following year the new, new master hub was created.
1982-D-25c-Washington-Quarter-Error-Obverse/I.g.w.t
Note how the date and the bust is different on the old/new dies?
With that the devices were not pushing into the rims. So these are just the way all of the quarters looked that year. Then factor in the die wear even making stranger looking examples. But when you examine the previous years, you can see the outer devices moving towards the rim.
