This is commonly called a " Dryer Coin" - what happens when a coin get's trapped in a dryer, usually commercial, and tumbled for hours if not days and weeks under heat, the rim will flatten out (like a tire on a car going flat), the coin will shrink slightly in size. I does look like someone may have used a grinder or a file to chop areas down as coinfrog states. Could be a combination of both.
