deliberate damage, either heated up, or some other type of damage applied to this coin, it didn't leave the mint this way..

yes,

My first thought was heat that caused the warping or possibly plating separation from the heat

Damage by heat, not a mint error. As silverwolf mentioned, it didn't leave the mint looking this way.
