Different roll of 1964-D, different RPM. RPM-004 is surprisingly wide (north) for a virtually unknown RPM. I doubt it's on the must have list for many collectors, but nice when you find one.

And maybe a small die chip on the number 4 in the date?

Right and wrong. Wrong is calling that thin lamination flake by the 4 a die chip. Right is that there is a die chip/gouge, but it's right above the D on the curve of the 6. It's bigger than it looks in the lighting I used, but it is a die marker for the RPM.

Looks like 1964-D RPM-004

1964D-1MM-004:

Note the marker on the bottom edge of the '6' is a match on your coin and the two sites. Wexler Doesn't show any RPMs for that year.

No images of the die chip on the '4'. If it were on the die with as RPM, it would be visible on all die states. It's not, so it was not on the die from the start. Thus I'm calling this a die chip.
Thanks fixed it.

Coop, you are correct with the link but it is RPM-004 not RPM-001. Thinking you have a typo there.
