I will say post mint damage ( PMD) done with heat. But let's see what others say.

I will say that @vest is right on with that answer. Rim dings aside, that bubbling in the middle seems to happen when clad coins are heated (such as in a campfire).

It looks to me that the quarter was stood up with the 19 of the date at the very bottom then heated up until the metal started sagging a little.
