Not an error. Extreme-post strike damage--the coin didn't leave the striking chamber in that condition. Coin was ground down--perhaps intentionally, perhaps being caught/trapped in a mechanical device.

You can't have a coin that was struck in collar with the rim only on side.
It has been intentionally damaged.
We have been seeing a few like this lately on this forum.
