They didn't really mint any 5 cash during the Qianlong era.

This one appears to be a H#22.203, minted around 1746-1753.

I just found what looks to be the same coin on numista but I can't seem to align the info marked on the sleeve. C#1-1.1 is what's written on the sleeve but I've had no luck searching it on NGC.

"C#1-1.1" would be the reference in the old Craig world coin catalogue - a precursor to the Krause catalogues that only covered the period 1750-1850. Craig didn't go into too much detail regarding the subvarieties and subtypes of cash coins, that the other catalogues referenced in Numista do. Numista itself doesn't go into this breakdown either.

Hartill is probably the best English-language catalogue for this purpose. It examines the minor differences in calligraphy that allow a slightly narrower date range than Craig or Krause allow for. As Wandering Circle said, this particular coin is Hartill variety 22.203, and can be placed in the period circa 1746-1753.

As for condition, I'd put it somewhere around Fine. I'm a bit worried about that grey spot at the bottom of the reverse (it's at about the 8 o'clock position in your second picture). I can't tell if it's just a stain, or if a piece of the coin has actually broken away.

It appears you are correct! I took the coin out of the holder for a closer look and there's is a chip off one side.
