Looks to be a classic Dryer Coin. Not an error, just lots of damage from knocking around in a washer or dryer for a long time.

I'm not sure if a dryer would carve that groove so perfectly (or dent and bend) around the coin on both sides. Maybe was in a bezel and worn on a chain - that would explain the smooth wear pattern on the interior of the coin on both sides.

Just a damaged coin. Once damaged, it really doesn't matter how it happened. (Out brains distract us trying to figure stuff out like this)
