This nickel appears to have been ground down when you look at the obverse, when you look closely at it though its clean cut all the way around and when you see the reverse its fully intact. any ideas how this occurred and is it a mint error. I've never seen anything like it. would it have any collector value?

Not a mint error, you can't have a full rim only on one side.
It has been modified, doesn't really matter how. PMD
No collector value for damage.

any idea how it was done as it does not appear that it was two coins glued together and the reverse is perfectly fine, the coin is normal thickness?

I agree with intentional post-strike damage. We've seen a few nickels and quarters with similar damage/features over the last few months.

Maybe clamping it down and using an angle grinder, and periodically rotating the coin?

Just guessing but it was probably prepared for jewelry and fell out of the setting, if it ever got that far.
