I have a 91 dime I found going through some rolls and was wondering what would or could cause an issue like this?

Nothing special for many years on Dime obverses. Check the sites to see what you should be look for on these:

Many years there are no listings after 1972. So they are pretty much a waste of time to look at the obverses post 1972.

Reverses there are some nice DDR's on the 1999 forward dimes. But look at each years listings to see what you need to find on these. other wise your just spinning you wheels. Train your eyes as what to look for. That will be a greater help on these post 1999 DDR's.
