I cannot imagine why someone would do this. Perhaps it was done at a timewhen these were not collected?

Looks to have been long ago--certainly was not uncommon in the 1800s and early 20th century.

Agree - engraving coins as love tokens was quite common.

Based on its extensive wear, It seems like that it could very well have been a piece pulled from contemporary circulation and made into a piece of jewelry / pendant.

I like the idea of using a pre-existing wreath as a frame for the inscription. It's clearly upside-down, though, which is awkward, but maybe the author did it deliberately to avoid connections to the old design.
(Alternately, there might have been a big reason to avoid having the portrait side upside down that outweighed other considerations.)

I agree that given the style and the level of wear this was very possibly a coin pulled from contemporary circulation.
