Question does a coin that exhibits md mean that all lettering shows md or can it also show only some lettering to have md?

Machine Doubling is mostly the product of the strike generally due to a loose die or stutter strike. It can be present on all or just some devices.

Note the Machine Doubling can show, apply or happen on Doubled Dies as well. Doubled Dies are formed or created during the hubbing process. Machine Doubling is created or a product of the striking process. Thanks, Doug.
