I know this 1991 is a mess, but wanted opinions on what would cause such damage. Don't see a mint mark.

Not an error but rather the zinc core is rapidly being destroyed by corrosion. This is definitely a spender (while you still can).

One of the more advanced stages of zinc rot I've seen on a Memorial cent. Excellent example.

I'm surprised it's still holding together at that stage.

Zinc is much more reactive than the copper a simple acid soak will not do this. They dissolved the copper first.
Someone did somethin a little more involved with this experiment.
Something like this https://goldrefiningforum.com/threa...penny.15960/

I believe you are over-thinking it, J-Tal. This coin most likely had damage in multiple areas (like a parking lot coin) that cause the zinc core to be exposed to air and moisture. Once that happens, as I understand, the copper and zinc work against one another (or together, depending on how you look at it) to cause the deterioration seen here. I don't know the name of the specific reaction that occurs but it probably has something to do with the exchange of electrons.

Kind of looks like a science experiment gone wrong.
There are many chemicals or substances which would cause damage like this.
Certainly zinc rot is the result.
