I have a Jefferson nickel the coin looks like it has a bad Planchette, the fields and some of the relief look rippled or melted and the Relief has some areas that are not fully stamped not flatted by wear just not fully filled in. Is this a bad planchette or something else.

To CCF! Your nickel is showing Die Deterioration. Your coin was struck with a worn out die.

A very strong case of Die Deterioration, yes. What does the reverse look like?

It is not an error - just a result of using the die too long.

May be slightly off center, but no enough to change the value.

When you see flow lines such as on your nickel, that is caused by Die Deterioration. It is not an error but part of the minting process.

Perhaps a misaligned die, but we would need to see the reverse to be sure.

The numismatic term is PLANCHET. A Planchette is used with an Ouija board.

yeah... that pair of dies was used, abused, put away wet, and brought out to slave away the next day and the next day and the next day, until they had the decency to crack up and be put out of their misery.
