Unfortunately, this coin has gotten it's share of dings, flattening, etc, etc. I cannot find any examples in V.V. (etc) which match this, but the separation at the top and front of the 4 has me wondering if it could still be a DDO, or just chalk it up to PMD. Sure wish it was better condition to tell a little easier!!
My vote is damage. If there was really that much separation of the top of the number 4 during the striking process, there would be other evidence of it elsewhere on that number and on other details.

Spence; Thanks for your input. I was hoping to find another spot with any sort of doubling, but especially with the condition, could not. Guess the top of the 4 was enough to send me to wishful thinking!
