Hi;
I can find no reference for a 1991 DDO, but:
This looks a little different to me than a normal DDD.
The inner curve of both 9's have very well defined outer edge, as well as the edge having a slight ridge. I suppose the ridge on the edge could be from the metal pushing beyond the die.
Thoughts on the coin(as well as my observation interpretation - just want to see if I'm at least starting to think in the right direction!)?

It looks like Die Deterioration Doubling. It's not a DDO.

Looks like more of a plating issue but regardless, not a doubled die.
