I am curious about the dent below the error in the lamination. Is it possible for a planchet to be clipped in this way? Looks like the Blakesley effect is present, and the dent/clip appears to be clean and smooth.

I see a small Clip and a Retained Rim Burr Struck Through. It's possible for a coin to be clipped slightly.

It looks to me like the "clip" is the piece of metal removed
from the rim burr. The missing section looks to be the
same shape as the rim burr .
Also a clip would be more straight down, not on an angle as shown.
It is possible to have a small clip, but I don't think this is the case.

I agree, the edge clearly shows a damage I don't beleive it is a clip neither.

Thanks for all the wisdom, spent the morning looking into strike-through rim burrs and rolling folds. This coin looks very close to one of the rolling fold examples on the following Coin World page.

It has some possible Blakesley effect happening on the opposite rim. Could this be because the planchet was slightly incomplete before the striking process? Or is it just coincidentally placed Die Deterioration or something of that nature?

JimmyD has some great point, Sure looks like the rim was damaged while possibly being placed into the striking chamber and the loose piece landed on top of the coin and it was struck into the surface.
