Just a straight business strike, the reverse die may have just been more worn than the obverse. Proof coins of that period were Philadelphia minted, so there wouldn't be a D mintmarked proof die.
Impossible to "accidentally" make a proof coin in error, it requires specially prepared dies, planchets, presses have to be set up differently, can't happen!
