It means that somebody improperly cleaned the coin at some point before it was submitted for grading. (this is why the statement "DO Not Clean Your Coins" can't be said enough!
That's not a "fake NGC" look; NCS is a subsidiary of NGC. NCS slabs coins which are not qualified for an NGC slab due to cleaning or alteration. I wouldn't go much over $25 for that coin.
RedBook values problem free versions of that coin at $80 in VF condition. So I wouldn't consider $50 for a cleaned coin to be an especially good value. In the long run you would probably be better off spending a little more for one without problems.
