I know this is not the best pic but once you see it I hope someone can tell me if this is a error or can tell me what it is.

The back of the coin has the same dots in the same places as the front.

and how would this type of thing be done with out heating up the coin?

I would have to say it's PMD,post mint damage. How it happened is anyone's guess. Maybe a perforated wheel like crafters use to perforate paper?
