Possibly an Indian Princely States coin?  What is the weight/diameter?

Yes, it's real. Your coin is a silver tanka in the name of the Timurid sultan Shahrukh, 1405-1447, struck at Herat in Afghanistan. The date is very weak but either AH 838 (AD 1435) or 848 (AD 1444). The side in the first photo has the Sunni Muslim kalima at the center ("there is no god but Allah, Muhammad is the apostle of Allah"). The petals around contain the names of the first four "rightly guided" caliphs of Islam: Abu Bakr, 'Umar, 'Uthman and 'Ali. The second image names the ruler: "the very great sultan Shahrukh Bahadur, may Allah preserve his kingdom and his sutanate". The mint name Herat is between the second and third lines. The date is at the very bottom.

Thank you so much for identifying it. I am very happy that it is a real coin (and not just a tourist token). It has been in every purse I have owned for the last 20 years.

As for what it is worth, considering it's history you really can't sell it now in my opinion. Like it or not you have created a good luck token for yourself having carried it for 20 years. You can give it away if you have a friend or family member who is in some poor situation and you can say how much this meant to you and how you think they should carry it now of course. Pretty much the best way to handle a good luck token and again just my opinion.
