1- Is it double Ear coin?
2- What is inside the letter "D" in word "GOD"?

1. This is most likely due to DDD

2. That is probably a die chip, considered an error, but too small for a premium

On the bottom of the lobe on the ear. Note that it is pushed down by damage or MD. Not a doubled die. On a doubled die the lobe would be doubled (enlarged), not altered in size.

I agree with Coop on the ear.
As to the motto: The entire motto is full of many letters that have been flattened or marked up by being hit. This dime has been circulated well. If you look closely at the "lump" on the D, you can see what looks like a "cupped" shape where the hit took place and displaced the metal.
