I know this coin is really beat up. But it looks like there is a 5 printed on the front. I've looked through many posts and I have not seen another one like it. Anyone have any information?

Welcome to CCF. You would need to overlay the "5" behind Jeff's neck of an intact image of a 1965 nickel. See if the size is right. It is possible that gunk filled up the 5 in the date, and then fell out onto the die. The next coin press would have what is called a "dropped letter". But a lot more research would need to be done.

Given the large amount of heavy damage elsewhere on this coin, I feel pretty confident saying that what looks like a number 5 is actually damage plus Pareidolia. As a matter of fact, my eyes were first drawn to what looks a little like a number 5 on Tom's neck rather than to the area further back on his ponytail.

I agree with Spence--a cornucopia of post-strike circulation damage.

Just an accumulation of damage over the years.
It didn't look like that when the coin was struck.
It's a spender.

I see a damaged coin, not sure how the metal formed to make that '5' but it is bigger than the 5 in the date. I think it is just coincidental damage.

I agree with everyone saying this is just PSD.

I'm not even sure where the OP is referring to as having a 5, since they didn't give a specific location.

Note the outside edge of the coin. It is not circular any longer. That is the clue that this is Post Strike Damage to the coin. Now just a face value coin.
