I agree with Coinfrog.

It is on Observe so it is an miss alignment. If was Reverse will be another story.

It is a tilted die issue. I feel these are caused by looseness of the machine. This one has a strong fin on it.

Most of the time they are thin and get flattened when they are rolled. This one has a strong fin.
