Just extreme die wear on both dies. During the use of the dies, you see the die wear affecting the outside edges of the devices that are facing the rim. That is the direction that the movement of metal is when striking a coin. Note how it is slightly seen as the die ages, until it gets to the VLDS die state. (Very-Late-Die-State)

The longer the die gets used, the more wear is noted on the coins struck with those dies.

Eventually the die wear get worse with the devices appear to move to a new location:

No DD 101 is a big one, you don't even have to have magnification to see it in the date.

Another thing to keep in mind, if you think you have a DDO, paired with a DDR, you should look at other possibilities, than a doubled die. It does happen but is not common. Any time I see someones thread saying they have a DDO / DDR, it is almost always, MD, Die Deterioration or lighting issues.
