It looks like PSD however the shape and the cuts don't seem to be very clear how could have been done.

Looks as if something heavy fell on top of it, since there is some corresponding damage on the reverse. It could have even been carried around by someone in the Civil War, and could have been damaged there!

It looks like it has been punched or hit with something, you can see the flat spot on the reverse from the impact.
In any case it is PMD

I concur with the likely deduction about the eradication of an obscene word. It's sad how many decent large cents were damaged by the alteration of the E in CENT. I once had a VG 1814 classic head cent that suffered this indignity.

Not all PMD is so detrimental though. Some counterstamped and engraved coins can be worth in the hundreds and thousands of dollars. Damaged coins that have been repurposed in a positive way are highly collectable.
