I could use some help on identifying what, if any, is going on for the date especially above the 9. The 1 and 5 look like die chips, but if I'm wrong please let me know.
Also, The mint mark looks like an OMM (D/P) but I'm still learning how to identify this error and I'm looking for someone who has more experience to help me here with this one.
Thank you.

Ahh....Yes, I see it now. I should've looked into that category off variety. Thanks. Still unsure of the 9 though. But, till I get someone who knows I'll keep trying to find out off Wexler's and VarietyV. Thanks for your help Coin Rejector.

You're welcome, glad I could be of assistance. I agree w/ your assessment (die chips on the numbers 1 & 5) but above the 9, it could be a tiny lamination? Lets see what others have to say.

Above the 9 looks to me like it was struck through a tiny piece of retained copper foil. Interesting but not sure it affects the numismatic value.

The first pic above looks a tad squeezed left-to-right. Overall, full, large and sharp pics of the obverse and reverse would help establish the context for what may be happening. All interesting.

Ok. I've uploaded a couple pics with adjustments on size for a better view.
