Looks to me that a penny was struck from a bad planchet.

Appears to be PMD, just a damaged coin

That coin has been through a lot in its time but I think it left the mint in good condition so it is just damaged now. The rim looks out of round where it is missing height, so I think it has been hit from a hammer type object.

What makes you think so, instead of just being damaged while in circulation?

it looks like chunks of copper might not have fully melted or a foreign material got into the planchet when the planchet was made or coin pressed. Look at the reverse, do you see a hole bunch of little holes in the copper?

consider this. In 1916 a penny was worth 1/8 of a loaf of bread. Not many people back then could say, hey the penny is damaged from the mint, let's save it. No they would spend it,

Just damage on this coin - the areas you identified in the last images are most likely caused by placing the coin on a rough surface then hitting the obverse with a hammer or something.

Whenever you have a gouge with a buildup of metal to go along with it, 99% of the time it's damage. It can't happen at the mint.
