Rim Error On 2013 D Nickel?

Hello

The rim on this nickel looks like something may have happened during minting process. Is it just PMD? Never seen the rim bent up like that before.

It's PMD, no way that could happen during the striking of the coin.
Something has hit the edge of the coin pushing the metal upwards.

That you will probably never know, But it was a perfect hit to shave that rim and push it up like that with barley touching the surface of the coin

Well, for sure we will likely never know, but that sort of scraping toward the center is sometimes found on coins previously used in vending machines, like bubble gum machines. Sometimes the rim is pressed centrally and other times the most peripheral parts of the inscription are. Considering the recent date of minting on this nickel, it seems less likely, but it is at least a possibility.

No way to tell for sure. But moving metal on a coin is not a rare thing. Simple...it was hit at just the right angle, the metal in the rim moved. Its a bit like trying to find out what specific object impacted a car to make a dent in it.
