Given the photos posted, it appears to be a case of Die Deterioration to begin with, followed by a long life in circulation--lots of wear.

Lots of circulation flattening along with Die Deterioration.

A lot of vending machine and circulation flattening on this coin. There are two DDO listed on this year, but not real strong examples:

So if it were DDO-001, the coin wear would have removed the hub doubling. 821+ million quarters that year. So they are pretty slim to find that year. They re ramping up to replace the silver Quarters that were being pulled from circulation at that time.
