It's not an error, it's damage.
"Dipping" is not considered cleaning... -from PCGS website

@rm, first welcome to CCF. Second, despite the heavy damage to this War Nickel, it does contain a bit of silver and therefore worth saving.

Rather brutal (and very likely intentional) post-strike damage. In my opinion, a neat find to keep as a curiosity.

Yep,  , most likely a possible damaged coin here. (done with diagonal cutters - wire cutters)

You are right to say ERROR, but it is the error of who masacrate this coin not to do a nice job. Sorry it is a hand made damage to the coin.
