I'm thinking post-strike damage on the 4--a lateral circulation hit which moved (soft) metal. We see 'moved metal hits' periodically on this forum.

It is not an error, it is damage from taking a hit.
Happens quite often when coins are in circulation.

While these look like coin damage, they are actually the working hub that is broken.
