Just a beat up nickel using tools. Maybe a doming block. Nothing in the minting process could produce something like this. It is just post mint damage.
The coin is cupped up on the reverse side - it also has clear glue on it. The obverse has been obliterated somehow. not too sure for what reason though.. just post mint damage now.
