For sure that is damage, but rather than environmental it looks more like mechanical impact to me—maybe a hammer? Are there corresponding flat spots on the rev?

Looks like deliberate damage to me as well.

Environmental damage would be things like rain water or dirt affecting a coin. Things like what you have here are intentional.
